چکیده

To determine the effect of thrombolytic therapy and/or immediate coronary angioplasty (PTCA) on left ventricularfunction. 129 patients with acute transmural myocardial infarction were retrospectively studied. Treatment strategies included thrombolytic therapy alone (n = 29), PTCA alone (n = 41), and combined thrombolytic therapy and PTCA (n = 59). Left ventricular ejection fraction (LVEF) and infarct zone regional wall motion (R WM) were determined from contrast ventriculography obtained acutely and at day 7-10. In the overall group, there was a 2 & 9% increase in LVEF (p < 0.02) and a 0.7 f 1.2 SD/chord increase in R WM (p < 0.0001) between day 1 and day 7-10. Patients with a patent infarct vessel at day 7-10 had a more significant change (A) in LVEF (3 f 8 vs -5 & 996, p = 0.0002) and RWM (0.8 f 1.2 vs 0.1 A 1.0 SD/chord, p < 0.02) than patients with .an occluded vessel. Patients with a residual stenosis < 70% at day 7-10 manifested a greater M V E F (3 f 8 vs -5 f 9%. p < 0.01) and ARWW (0.9 k 1.2 vs 0.1 1.0 SD/ chord, p < 0.05) than patients who were occluded. There was a negative correlation between residual

Myocardial damage of the entire ventricular region in a patient with acute myocardial infarction

Technetium-99m-pyrophosphate (99mTc-PYP) has been used, in combination with thallium-201, to estimate the site and extent of myocardial infarcts. We report a case of acute myocardial infarction with severe coronary disease in which the distribution of 99mTc-PYP was extensive. A 78-year-old man presented with dyspnea, and a diagnosis of non-ST-segment elevation acute myocar...

Coronary bypass surgery improves global and regional left ventricular function following thrombolytic therapy for acute myocardial infarction. TAMI Study Group.

Coronary bypass surgery was performed prior to hospital discharge in 303 (22%) of 1387 consecutive patients enrolled in the TAMI 1 to 3 and 5 trials of intravenous thrombolytic therapy for acute myocardial infarction. Bypass surgery was of emergency nature (less than 24 hours from treatment with intravenous thrombolytic therapy) in 36 (2.6%) and was deferred (greater than 24 hours) in 267 (19.3...

Left Ventricular Function After Primary Coronary Angioplast Compared With Intravenous Streptokinase in Acute Myocardial Infarction

Background Early and effective flow through the infarctrelated vessel is probably of paramount importance for limitation of infarct size and preservation of left ventricular function in patients with acute myocardial infarction. Primary coronary angioplasty may offer advantages in these respects compared with thrombolytic therapy. The purpose of the present study was to assess the effects on es...

Randomized trial of direct coronary angioplasty versus intravenous streptokinase in acute myocardial infarction.

OBJECTIVES The objective of this study was to obtain preliminary data on the relative clinical utility of direct coronary angioplasty compared with that of intravenous thrombolytic therapy for patients with acute myocardial infarction. BACKGROUND The relative merits of intravenous thrombolytic therapy and direct coronary angioplasty as treatment for acute myocardial infarction are incompletel...

Acute intervention during myocardial infarction in patients with prior coronary bypass surgery.

T hrombolytic intervention with or without coronary angioplasty is becoming widely accepted as standard therapy for patients who present early with acute myocardial infarction, Thrombolytic therapy has been shown to improve left ventricular function* and reduce mortality.2 Nearly all clinical trials of myocardial reperfusion have excluded patients with a history of coronary artery bypass grafti...
